Comparison of Finbrial Eversion Methods in Operative Pelviscopy for Hydrosalphinx



Abstract
In 30 patients who was diagnosed obstructed bilateral hydrosalpinx by hysterosalpingogram, the present study was undertaken to evaluated and compare the efficacy of endocoagu lator and suture¢¥ in eversion of terminal salpinx after incision to clubbed hydrosalpinx.
Incision was performed by low current needle microdiathermy in endocoagulator group and by microscissor in suture group. Postoperative tubal patency was evaluated 1 month after operation by hysterosalpingogram or second look laparoscopy.
Of the 30 cases, 16 cases of fimbrial eversion was done with endocoagulator and 14 with suture. Using AFS criteria, the group was reclassified into 3 groups by the degree of pelvic adhesion and tubal obstruction ; mild, moderate and severe.
Postoperative hysterosalpingogram or second look laparoscopy documented 8(57.1 % ) cases of tubal patency on 14 sutured group and 13(81.3%) on 16 endocoagulator group and they are significantly different(p<0.05).
The tubal patency rate of mild, moderate, severe cases in endocoagulator group was 3/3, 5/5, 3/8 and suture group was 2/2, 3/5, 3/7, respectively.
Our study suggested that better tubal patency rate was obtained by endocoagulator technique than suture. Moreover this method was easier to conduct and less operative bleeding than the suture method.
Any firm conclusion on pregnancy rate between 2 groups needs longer follow up.
